The borang JKKP forms and when each one is used
Section 32 of Act 514 says an employer shall notify the nearest DOSH office of any accident, dangerous occurrence, occupational poisoning or occupational disease. That applies to what has happened and to what is likely to happen at the workplace. The NADOPOD regulations of 2004 turn that duty into forms.

Four points do not appear on the form itself.
- A death, a serious bodily injury or a dangerous occurrence is reported to the nearest DOSH office immediately by the quickest means. The written JKKP 6 follows within seven days.
- Those seven days are seven working days, and the deadline applies to the JKKP 6 itself.
- Everything that goes on a JKKP 6 or a JKKP 7 also goes into the JKKP 8 register, and so does every smaller case that was never notified.
- If there were no recordable cases in a year, tell the Director General in writing with your company details. Do not send an empty form.
The JKKP 9 and JKKP 10 are not routine. They are the formats DOSH uses when the Director General wants more detail about a case, which means you have to have done your own investigation first.

Counter hours are the same in most of these offices. Monday to Thursday they run from 8.00 am to 1.00 pm and from 2.00 pm to 5.00 pm. Friday has a longer midday break. The offices are closed on Saturday, Sunday and public holidays. Check the DOSH contact page before you travel there, because the payment counter closes earlier than the service counter.
The records that sit behind the forms
A notification is one piece of paper. What DOSH can also ask to see is the file behind it, and that file is where equipment paperwork lives.
- The Industry Code of Practice for Safe Working in a Confined Space 2010 says a report on every examination of entry equipment shall be kept available for inspection. Closed permits to work are kept for at least one year.
- The same code says other documents, including inspection reports, risk assessments, training records and rescue plans, are kept for inspection by the Director General.
- DOSH's HIRARC guideline says assessment reports are kept for at least three years and reviewed every three years, or sooner when a process or an activity changes.
